1-Hour Scaffold Card (in-person portion)

If you have completed the 4-Hour Supported Scaffold User and Refresher SCA-201 or SCA-B201 online, you need to complete the in-person portion of the course to receive the NYC Scaffold User Card. You need this card in order to work on, or use a scaffold in New York City, as per Department of Building regulations.

IMPORTANT NOTICE: As per New York City Department of Buildings directive from 28 May 2020, online portion of the course is only valid towards the NYC DOB Site Safety Training (SST) card. This online course alone cannot be used to obtain the 4 Hour Supported Scaffold User and Refresher Card. To obtain the scaffold card you need to take the 1 Hour in-person part of this course as well. • Supported Scaffold Use
• Supported Scaffold Inspections: Equipment & Hardware
• Emergency Situations & Preparedness Procedures

To receive your NYC Scaffold User Card you need to submit the following document to info@pdhstar.com:

• Copy of your certificates of completion for the 4-Hour Supported Scaffold User and Refresher SCA-201 or SCA-B201. If you have done your online training with PDH STAR in the past five (5) years you do not need to submit your Certificates of Completion
• A photo conforming to the DOB photo requirements (see below). A simple passport-style portrait photo against a light colored surface (e.g. a wall) would suffice
• A copy of your ID showing your current address – if you do not have a NY State issued ID you will need to submit other approved document(s) (dated in the last 60 days) as a proof of address (e.g. utility bill) . For full ID and proof of address requirements please click here
• NYC Scaffold User Cards are processed in the order they are received. If you have done your online portion of the course with other providers, the response time may vary, depending on the responsiveness of the provider where you took the courses. In general, NYC Scaffold User Cards are typically issued within 5-7 business days from receipt of all documentation and payment.

NYC DOB Photo Requirements

Photo Requirements:

• in color
• taken against a plain cream or light grey background
• clear and in focus
• no more than 2 years old
• unaltered by computer software

The image should:

• show a close-up of full head and shoulders without any other objects or people
• be between 29mm and 34mm high from the crown to the chin (we will crop this)

In addition the photo should show the card applicant:

• facing forward and looking straight at the camera
• with a neutral expression and mouth closed
• without anything covering the face
• in clear contrast to the background
• without a head covering (unless it’s worn for religious or medical reasons)
• with eyes open, visible and free from reflection or glare from glasses
• with eyes not covered by sunglasses, tinted glasses, glasses frames or hair
• without any ‘red eye’

Refund and Cancellation Policy

All NYC Scaffold User Card sales are final, no refund is possible. PDH STAR reserves the right not to issue the scaffold card if your training cannot be verified or does not meet NYC DOB requirements, or your ID and proof of address do not meet NYC DOB requirements, or cannot be verified. Please note that you need to attend 100% of the in-person part of the course to be able to receive the NYC Scaffold User Card.
