Wallet Size Training Card

If you have completed any of the OSHA Compliance or other training with PDH STAR, you are eligible to receive a training card equipped with a QR scan code to verify your training instantly. The QR code can be scanned with any smart phone and will serve as your proof of training, even at a construction site. You can order the card here and in the Additional Information box at the checkout please include the following information:

• Name of trainee
• Name of the course taken
• Date of course completion

Cards can be ordered for any regulatory or compliance training, such as OSHA, ANSI, etc. Once the order is received, you will be mailed your wallet size training card within 3 business days via USPS standard mail.

Refund and Cancellation Policy

All wallet size card sales are final, no refund is possible. PDH STAR reserves the right not to issue the card if your training cannot be verified, or you failed to upload your government issued ID during the course enrollment process. You may have to provide a digital portrait photo meeting ID quality standards taken against a light background.
