How to Request Your SST Card

Simple and fast procedure for ordering your Site Safety Training card

SST Card is a mandatory training requirement to work on most construction sites in New York City, and is a proof that you have completed your training with an approved Department of Buildings training provider. PDH STAR is a New York City Department of Buildings (DOB) approved Site Safety Training (SST) provider and is authorized to issue the mandatory Temporary, Site Safety (Worker) and Supervisor cards. We can issue the card even if you have completed your training with other course provider(s).

Please note that, in case your application for an Site Safety Training card is denied, you will not be able to receive any refund – for trainees who completed an SST package (10, 22, 32, 40, or 62 Hour Package) with PDH STAR, the card is processed free of charge. All Site Safety Training card orders are non-refundable. It is your responsibility to make sure that you are eligible to receive the card and have all the required documentation before making the payment. In case you have completed the training with another course provider, you need to submit all supporting documentation as described below, including a copy (front and back) of your OSHA card – OSHA certificates of completion issued by other providers are not acceptable for the purpose of issuing an SST card.

How To Request An SST Card

Once you complete all of your training, you need to let us know that you have completed your training, and submit the following documents via email (info@pdhstar.com) in PDF or JPEG format ONLY (no external links please, we will not download any documents from external drives):

  1. Site Safety Training Card Application Form: To request your card please fill out the form (download form here) and either email to info@pdhstar.com.
  2. OSHA Card: In order for us to verify your OSHA 10 or  OSHA 30 training, you need to email us a front and back copy of your OSHA card, Please include this with your application. Temporary Certificates of Completion from other providers are not acceptable. OSHA cards older than 5 years cannot be used for issuing your first SST card. You would either need to repeat your OSHA 30 training, or “refresh” your OSHA 30 card by completing additional 8 Hours of SST training.
  3. Training Verification: If you have completed any of the training with another training provider, you must submit your Certificates of Completion for verification (PLEASE DO NOT SEND CERTIFICATES OF COURSES YOU HAVE COMPLETED WITH PDH STAR). PDH STAR needs to verify your training taken through other training providers. For additional details regarding the training, including online training, verification, please click here. Individuals presenting Certificates of Completions or other documentation as proof of their online training are required to sign a document attesting to having completed the training on their own and according to the law.   4. ID and Proof of Address: For us to be able to issue the card, the DOB requires us to verify identity via a government issued ID and a proof of address. For eligible types of documents please click here. Please submit a copy of your documents with your application. In general, a valid New York State issued driving license or ID document would typically satisfy these requirements. If you have an out of state driving license you would also need to submit another document as proof of your address, such as utility bill, cell phone bill, bank statement etc. dated in the last 60 days.
  5. Photo: You need to submit a clear, passport like photo taken against a light background, with no headwear, shadows, etc. Typically, a simple portrait photo taken against the wall would be acceptable. For detailed DOB photo requirements please click here.
  6. For Upgrades and Renewals – submit a copy of your current SST card
  7. Confirm your mailing address where you want the card to be mailed

Ordering Your SST Card


If you have signed up for one of the SST packages (10/22/32/40/62 Hours), your first card (Temporary or Permanent) is included in the price of the package. If you have done only partial training with PDH STAR, or have purchased the courses individually, you need to order your card by clicking on the relevant link below. The card processing fee is:

Supervisor SST Card                             $75

Site Safety (Worker) SST Card           $50

Processing time varies, depending on which provider you have taken your training with, so please allow sufficient time for your training to be verified and the card type to be issued. Please make sure your application is completed in full with legible block letters – incomplete and / or illegible forms may delay your card issuance.

If your card is not received due to an incomplete or inaccurate address provided, theft, or any other circumstances outside of PDH STAR’s control, the applicant shall pay an additional $50 reprint and processing fee.     If you need to order a replacement SST card (due to card being lost, stolen or damaged), you can order it here. If you need to change your name on the card, the replacement card will be charged at $50. Please make sure that you spell your legal name correctly to avoid unnecessary delays and costs. The purchase of the replacement SST card is non-refundable.
